Two practical notes on the team. First, every role needs a primary and a backup, including the Decider — crises do not respect vacation calendars. Why it works: describes the harm, owns it, names a cause, names a fix, gives affected users a path. Avoid “we apologize if anyone was offended” phrasing — non-apology apologies are reliably worse than silence.

Correction (factual error in coverage)
